| Dear Parent or Guardian,
The Trailblazers trip to Camp Soles is rapidly
approaching. In an effort to make the experience a healthy one, I am asking
your cooperation in providing adequate preparation for your child’s health
needs while at camp. Please note the following important items and contact
me accordingly.
 | Emergency cards that were completed in September
will be taken to Camp Soles. If there are any
changes to your child’s emergency card, notify me in writing.
Such changes would include home, work, and emergency contact telephone
numbers or changes in medical conditions. |
| Prescription medications must abide by school
district policy. A written physician’s order plus a labeled pharmacy
bottle and written parent/guardian permission are needed.
The paperwork currently on file for this school
year may not be sufficient for Camp Soles.
Contact me
immediately if your child needs prescription medication while at camp. |
| Over-the-counter
medications must be provided by the parent/guardian of the student.
If you want any over-the-counter medications to be available for your
child while at Camp Soles, it must be in the original container with
written permission from you. |
| No student is permitted to
carry medication of any kind. All medications will be kept
locked. Medication will be administered by the school/camp nurse.

All medication and information must be
received by Monday April 28, 2008.
